Whether ICAI prescribed any rule / discipline regarding mentioning membership number of a CA on power of attorney / Letter of Authortiy / Vakalatnama . If yes , please quote the relevant circular / order number .

Never heard of any such revision for Vakalat. You can use the prefix CA at max in POA.
