Yes, you are liable to file ITC-04 for all the materials  sent to job worker.  It is immaterial whether job worker is registered or not.

ITC-04 need to file quarterly on or before 25th day of the month suceeding of that quarter.

In case of non-filing, there is provision of penalty of Rs. 200/- per day

As per rule no 45 Sub rule3 , The details of challans in respect of goods dispatched to a job worker or received from a job worker or sent from job worker required to file ITC04 for a period on or before the 25th Day of the month succeeding the said quarter.
In our city entire process is of job work, nobody is filing Itc 04, it is impossible to maintain so much of compliance.
great dear anil Lahoti,

do approach to GST council to make the separate GST rules for your city
Our association convinced them of job work profiles, morover no revenue by this form... still waiting for their reply
