When I paid the examination fee today I got the PDF file and it has 3 pages. I am a CPT route student so i think i just have to send first two pages. Do i have to send the third page as well ?

Yes sent all 3 Pages.
You mean i should send the 3rd page blank cause it is regarding itt & orientation and i have not done it yet

Yes, you need to send all 3 pages without which your form will be rejected . ITT and Orientation courses will be conducted later but you have to register for them now . Do not send any blank page.And check the form again, page 3 doesn't asks for information regarding ITT and Orientation. When you register for IPC, you automatically register for the above mentioned courses.
