There is no standard format for Internal Audit reporting. You can design your own Internal Audit Report format which may include following item related to issues to be reported

Impact analysis
suggestions

There being no fixed format for any Internal Audit report.However one must keep in mind to include the Action taken reports also.Besides the general Observation,suggestions and impact on the deeds and emphasis shall be on controls and its benifits.Revenue leakages also shall be given highlights
