Help CWA Yellow Cover Sheet for CC paper wanted

I'm a working professional from a science background and completely new to ICWA. I have registered for De'11 inter attempt and am strgulling to find th answer to a simple question.

Is there a prescribed standar sheet size/format to be used to answer the cc test papers.

How do we get the Yellow cover sheet mentioned on thie thread. I did not receive anything with my material.

Request urgent help.
