whether medical treatment provided by hospitals for fracture or any other sugery attract gst for its employees if the bill is raised in favor of company?

No. GST is not levied over any medical treatment, as health care  service is exempted under GST act; but the contract with company need to be checked for the services.

If the value of the contract outsourced is small then no gst.
We do not have contract with the hospital. the employee has availed treatment in a hospital and company is paying directly to the hospital rather than the employee so is it covered under gst

No. No GST applicable, when employees are treated without any contract.

In my view As such it is not block Under Section 17(5) so ITC cam be claimed.
Moreover if it obligation of employer under any law then definitely you can claim ITC.
