pls send me noc format for registraion of service tax department

Sirs, I am the proprietor and the business premises is in co-owned by my husband and his brother. Is it suffice to submit sale deed and noc from both the owners in my favour or what other documents are required to be submitted for service tax registration. Kindly guide.
