form 16 A

Form 16A is basically a TDS certificate. It is for income other than salary. In cases of for eg, bank deducting TDS on rent receipts. It contains details such as name and adress of the deductor, PAN and challan details too. Apart from that, it also has details regarding the income that you've earned and the TDS deducted and deposited on such income.
