Exam Form

I had taken termination from articleship n had completed 11  months of training. ..so how can I apply for may 2017 exam form...n to whom I get cos signed from n is that ca is eligible to sign in place of principle  n give assurity of completing 9 months articleship. ...please suggest d whole proceedure that how should  I apply 

guys , i have downloaded the pdf after filling my exam form and my photo and signature and address are appearing on it , but still on the 2nd page it mentions a blank space to write the candidates name and fathers name on the first line and a blank space for members sign at bottom right , plz clarify asap
