Above replies are totally incorrect, Mr. Sourav and Virendra if you don't know the answer then don't give wrong answers. Did you know that If you have two Pan Card then there is penalty of Rs 10000.
Mr. Naresh Ji You just go to income tax department and contact your jurisdictional officer. or take the help of your nearest CA, they will help you.
Agree with Mr. Ankit , above replies by both the persons are wrong, that is not a solution, if they don't know the answer why are they replying, just for fixing their spot at no 1.
1. Write a letter addressing the Assessing Officer (A.O.)for reactivation of deactivated card 2. Attach self attested copy of the deactivated PAN card 3. Execute the indemnity Bond in favour of Income Tax Department
Ankit ji if you know the answer so well then why dont you give relevant section and rules providing procedures for renewal of PAN.
In absence of specific rules for renewal we had no option but to create our own solution. and i doubt if penalty for two PAN will attract in this case.

Mr Virendra I am not diverting , that is a practical solution ok, I am practicing CA, I never give wrong advice. Penalty may attract if you have two pan Card. Card is deactivated that can be activated by department.
There is no section in Income Tax Act to activate PAN card which is deactivated Income tax department has deactivated more than 11 lakh duplicate and fake PAN cards, there might be instances where a genuine PAN was deactivated
