i want to know that when exam form of CS will come and whose attestation is required for that. can we get attested from any CA?

cs form will come in march & september of year and last date to fill is 25 mar and 25sep of a year for attempting an exam in june &december of a year.
25th march (with late fee of rs 100 till 9th april) for June Exam
25th sept ( with late fee of Rs 100 till 10th oct) for Dec Exam
Examination fee for Professional Progrm Per Module Rs. 750..
