Ca final exam application

I have applied for Nov 2014 CA Final Exams. But did not attend the exams.

Now while applying for May 2015 exams, should i mention Nov 2014 in Last appearance column in application form? or should i mention the earlier attempt i wrote in Nov 2013 as my Previuos Apperance?

Moreover i dont have the roll number now for Nov 2014 exam. How to search it now?

Please help me in this regard.... I am waiting to apply
