I have completed my 42 months pcc training and now I have to fill the form for quiting the articleship. I just need to confirm that do we have to give a bank statement to the institute for showing the total cheques we got or what is the actual way for the termination?

no bank statement is required to be attached. only A/c. no is to be mentioned. even that is not a necessity . only when there is dispute and there is need to show that article has got stiphend then he need to show bank statement as proof .
My articleships is getting over next month i.e on 20 september 2010....and during the 3 and half year articleships i had once taken transfer and there i had around 45 days leave....so i wanted to know that while filing the 108 form in next month do i have to include the leave which i had taken under the previous employer....or i have to just show the leave which i have taken under my present employer....plsss reply soon...Thank U
