After ca final examination

The Institute sends letter to CA Final examinees who are in their last 6 months of articleship informing them that their admission to CA final exam is on a provisional basis. You just have to fill up the form that came with the letter and get it attested by your employer and send it to the HO (Delhi / Noida).

I have received only a letter but no form is attached to it..what should I do??

I too recd only letter no form attached wid it.
