Written notice for transfer of shares from company

I am into a private limited company and processing a transaction of partly gifting shares from one director to another. As per the transfer procedure,we received application from directior for splitting his share in to 2.Comany has accepted the request and passed the board resolution for the same and now in the process of sending notice to transferor with the valuation of share.

I just want to know the legal format of such notice to forward to transferor.I think in this notice itself company  request to tranferor to submit share transfor form with stamp duty and gift deed also on stamp paper.
