Which itr form to file for salaried doctor receiving payment under section 194j

Hi.. I'm a doctor working with a private diagnostic centre. I receive fixed remuneration in the form of professional fees ( under section 194J) and Form 16a. No other source of income. Annual income is less than 20 lakhs. Which ITR form should I fill?

1. If TDS is deducted u/s 194J then the income is in the nature of the profession and generally, ITR-3 can be used. 
2. But if you prefer to opt for presumptive taxation u/s 44ADA then ITR -4 can be utilized. 
Please correct me if the above solution has an alternative view.

Being a doctor, you can file ITR 4 claiming deduction under 44ada
