No, is vajah se form cancel nhi hoga. form me specially mention h ki hum form blue pen se v bhar skte h bt black pen preferable h. so dn't worry.

1. Ur reg no.
2. Medium
3. Both/single group
4. DD (Amt Rs.)
rest all (incl.blue/black ink) do not even bother. Get back 2 books.

It doesnt matter whether u use blue or black ink, unless u hve filled the following info correctly.....
