Schedule vi-a. 80d (a). should i enter premium amount of total basic premium?

The insurance policy shows in a table, the name of the person insured etc, and mentions that "Medical Premium" in that row as Rs.5196.

Below that it lists

Premium computation:

Total basic premium: Rs.5716

Net Premium: Rs.5716

Service tax (14%): Rs.800

Swachh Bharat Cess (0.5%): Rs.29

Krishi Kalyan Cess (0.5%): Rs.29

Total Premium computed: Rs.6574.

The total amount I had paid was Rs.6574, but I'm confused if I should enter 6574 or 5176 or 5196 in 80D (A)?
