If you are applying for Government jobs usually the passport size photo is affixed otherwise not required. I think you meant Declaration at the end of your CV. It is your discretion and normally corporates have their own back ground verification teams to cross check an employees credentials. Other organizations which can't afford background checks simply rely on trust.
