Resignation of director due to ill health

Question 1:    I have to file DIR 12 towards resignation of Director due to ill health or sudden illness. so in DIR 12 there is a box in which reason for resignation need to be filled, for example: there are option such as death, retirement, disqualification under section 164 etc... so what will be appropriate section for such resignation, and what will be the ATTACHMENT

Question 2: same way i have to file DIR 11 towards resignation of director due to ill health, what will be the ATTACHMENT as (Notice of resignation and Proof of Dispatch is mandatory).
