Im disallowing TDS not paid till date of Audit Report i.e. 30.09.2011 in cl 17 so i also have to mention in the annexure of tds i.e. tds not paid till date???? becz tds paid on 15/11/11

You have to mentioned it under the details (iv) tax deducted but not paid to the credit of the Central Government.
See this clause give the details that TDS has not been paid upto the date of issue of Audit Report.
