I have sold goods to a dealer in Bhutan against exemption. I am based at west bengal
What are the formalities thereafter ? Who will issue the Form-H to whom ?

Dear Mukesh,
Since you have a direct export you must produce the bill of lading, order copy of the importer of that country, and your export invoice, you need not produce any form H from others. In fact you have to give the Form H to others from whom you have purchased the goods for exports
