Query regarding vat

I had general query regarding vat liabilty in case a doctor prescribes drugs to his patients in a clinic. In this case is he liable to charge vat on the amount of medicines given or does it come under service ?

What if the doctor charges one specific amount for both the medicine and the consultation fee without any segregating the amount of medicine given and the consultancy fee.
