I have a query regarding the presentation of shareholder information as per Revised Schedule VI. Can anyone tell me whether in a case a shareholder holds exactly 5% of the equity share capital his details are required to be disclosed in the Notes to the Balance Sheet. The language used in the instructions for Balance Sheet preparation says "6. A company shall disclose the following in the notes to accounts : (g) shares in the company held by each shareholder holding more than 5 per cent shares specifying the number of shares held".

I think only more than 5% has to be reported i.e. holding exactly 5% is not required to be reported

You already have the answer to your query. You are absolutely right disclosure is required only if shareholders holds more than 5% shares
