PF rejection: Asked to re-apply in prescribed proforma

Dear Members,

Kindly help me with my problem. 

. I’m trying to withdraw my PF amount for purchase of a house. I applied online using Form 31. My claim got rejected with the following errors.

For Error 1) what is the prescribed proforma in which I should reapply for the PF advance. Can you help me?

For Error 2) I do not have a passbook as I hold an account with HDFC bank. I uploaded a cancelled cheque. Who is the authorised signatory for a cancelled cheque. Should I sign it, or should I get the bank to sign it? I have attached my Form 31 that I submitted for reference.

Kindly help me resolve these two errors.

Also, after I submit the Form 31 online, do I need to submit any additional documents to PF office offline? The error message is very cryptic and I do not know how to resolve this.

Also, I have raised a grievance for this issue but there is no response to this.
