Name change in pan after marriage

You need to submit Annexure A along with DOB proof.

Name change in case of Married ladies -

Marriage certificate /marriage invitation card/publication of name change in official gazette/ copy of passport showing husband’s name/ certificate from a gazetted officer.

Please reffer guidelines of NSDL.

They want to change her name as Pavitra Suresh instead of Pavitra K.

Hence, Better use Annexure A for such change of name.

Transfer Of Ownership make LEGAL now.

Does she have her marriage invitation card ? if yes then you can use it along with documents like -

Application for correction, Old PAN copy, ID proof (new name after marriage),  Address proof (new name after marriage), for date of birth her matriculation certificate can use (whether it contains name before marriage or not ) & additional document her marriage invitation card .
