Last date of conversion from PE2 to IPCC

To all dear friends

its a kind request pls pls help

i want to appear in nov 2011 ipcc exam

i hv already cleared group1 in pe2 and also completed my articleship

i hv gone through the procedure of conversion but still i m confused

pls tell me when to submit conversion form if i want to appear in nov 11 exam?

do i need to finish 100 hrs itt training before appear in exam?

and now as on today how much fees i have to pay for conversion?
