ITS URGEND...NEED UR HELP

the asssessee have received the mail mentioning username and password credentials.....bt when assessee tries to login or create his own username and password than message comes that ur provisional id is already mapped to username.......bt when assessee click on forgot username option than message comes that username is not created......please tell me needful action as due date of form 3B is 20th augest.
