Dear Sir, I have to apply for IPCC revalidation as it has crossed the validity period of four years. I took the DD of 400 rs and went to Bangalore branch of ICAI. When enquired about the Revalidation form i was told to go through icai website.. But no where i am able to find the Revalidation form.Can u plz tell me if there is any prescribrd form for IPCC revalidation or should we write it in a paper by our own with the format given in ICAI website.? And also to what address it has to be sent? Plz help me out. I am tensed. Thank u in advance

The request for revalidation of registration shall be made in the application format as prescribed by the ICAI in its announcement dated 22.01.2013.

Fill the form and send it to ICAI office and attach a photocopy of same while submitting exam form

I have completed articleship and attending Nov 2016 ipcc exam ,in exam form is that necessary to fill and sign Certificate of Service ? Kindly inform me.
