Hi Ajay i am shreyans i filled the form on 30th july and we have to fill the acknowledgment for sure where in the coloumn "drawn on" you have to write the name of the bank .

i hope rest all is quite easy to fill

but in the coloumn ,drawn on i wrote my name,,,,,??????? will i face any problem

Yes, you might , so i would recommend if the form is downloaded from internet then download another one and print only the page containing acknowledgemnt and fill it again and replace with the older one or you can also do one thing, you may use fluid but i would'nt recommend that coz that might create some issues after submitting.
