i am incorporting a new pvt limited company with authorised capital of rs 100000 in which 50% shares are holded by a foreign national subsriber and after submition of form 1 on mca portal it was marked for resubmittion and the following remark was given by MCA "2nd prom. of moa/aoa is f/national but copy of passport/visa and affidavit from the professional has not been scanned"
please guide me what to do and provide me format of affidavit if any

It is very simple & easy. Take note of the following:
1) Photo copies with self attestation of Passport.
2) Photo copies with self attestation of Visa (that is his residence status such as Driving License, Income Tax Card or Permanent Residenceship Card issued by his Country's Government).
3) The foreigner should approach a Notary Public of his nation or jurisdiction were his residence is located. That is, signed by an authorized person by the Court of his nation with seal placed in red color wax.
4) Then the above mentioned documents has to be scanned clearly in .PDF format and uploaded.
MCA would then accept and approve the same.
