If you are applying for PAN of a HUF then simply state the name as " Sudhanshu kumar HUF".

For Date of incorporation you can give DOB of the karta only or any other date on which the HUF derived some property. They dont ask proof for that.

For contact you can give the residential details/office detaails (either one is enough) as the place of communication
