GTA Services-How Bill to be raised.

We are registered as a GTA. We are dealing with registered company. Our concern : Mr. A is the company owner gives the goods to us to deliver it from Tamil Nadu to Delhi. We pick the goods by tempo and reached the Central Railway station and booked the goods to Delhi, there's our own employees, cleared the goods from Railways and deliver it to the concern person by hiring local vehicle. Here, the Railway charged us 5% as a tax, and how we have to raise the CONSIGNMENT note to our customer. Pls do the needful. Thanks
