my Client has medical shop registered under composition where no proper accounting is done.
we are paying 1 % of the turnover.
pls guide whether it is mandatory to give purchase details in GSTR 4?

Yes , you require to give purchase details in GSTR 4 .
But the purchase records are not maintained
Yes it is mandatory for all composition dealer
But sir Notification no. 60 of 2018 says that no need to put the details of purchase from registered persons
From 1/4/2019 it is mandatory to give purchase details .
Also you can check bank entries to find out purchase details , as you may have paid through bank to your Creditors . Also check cash payment if any record .
