My airticlehsip is going to complete this month suggest me which form to be submitted ....and also i have served 1year extension of airticle so which form for extension ...and is there any penalty pleas provide full deatails i need it urgently..........

You need to submit Form 108 to the Institue
No need to submit any others forms for Extension of Service, all you have to do is, fill the leave availed and No of days served including Extended Service, make sure that it would covers the actual period to be served.
No penalty for extension of service
