Form 60 submitted during saving account opening in 2005 whether it's valid today or it is valid for only that year

After the notification of mandatorily linking PAN........ form 60 filed in 2005 became invalid...
When notification of mandatory linking issued
1st January; 2016.....
OK incase form 60 filed during 2015 in bank account
Not acceptable as per RBI directives.......... PAN mandatory.......
But sir bank still accepts form 60 for account opening
Not Normal savings accounts......... that is only for Jan Dhan accounts......
So form 60 for normal saving account is invalid
At least after January, 2016........
form 60 accounts total transaction below 10000pm
10000 pm what is this calculation. I think this limit is for basic saving account
