YOU SHOULD MAKE PAYMENT BY DRAFT ON THE FOLLOWING NAME :-

"THE SECRETARY, THE INSTITUTE OF CHARTERED ACCOUNTANTS OF INDIA" PAYABLE AT.......................(ACCORDING TO REGION) ALONGWITH CONDONATION LETTER..

You need to file 112 only if you are doing articleship.

And the amount of penalty is determined by ICAI
