Form 108

Dear experts,

I will be completing my articleship on 31st March 2011, I wanted to know few informations regarding Form 108

1) I have finished my articles in Bangalore, what will be my mailing address for sending Form 108?

2) Is there any requirenment to fill any specified number of copies of Form 108?

3) If you submit the form within 30 days of completion, are you suppose to pay Rs 100/-, If yes what are the details to be submitted in DD?

Please help me in this regard.
