EPF form for not deduction.

the employee has to submit form 11, it is mendatory , and if he has any past EPF no /YAN no active during any service, he has to declare and participate in EPF on mandarory basis, 

However if the past EPF column is NIL or Not Applicable then, he can give declaration that he does not want to join EPF as his basic is over 15000/-  employer may follow that form 11 declaration  

if any active UAN is found in name of present employee then employer will have to face troubles so he must have to obtain form 11 with declaration before allowing grace from EPF contribution
