A Company Status (for efilling) in Master Data: "Dormat", Please provide me procedure to revive the Dormat status as Active. No DIN-3 has filled. Now kindly confirm me the step by step procedure to active the status and continue filling.

File Form 61 for normalise the company .
Form 61 should have an application and Board resolution. On approval by ROC of the said form all pending forms has to be filed within 21 days.
Kindly provide me a sample format of an application to be attach with form 61 and sample copy of resolution
