Difference between u/s 80d(01) and 80d(04)

my employer has mentioned in form 16(b) under head deductins under chapter Vi-A under 80d(01) an amount of Rs 9887 and under head 80d(04) and amount of Rs 5952.what is the diffrence between 80d(01) and 80d(04)?

my father is an independent senior citizen and i have taken the mediclaim policy  for me and my father with my employer.will i be able to claim deduction for the premium amount paid for my father too?
