Condonation of delay

I want to file an Appeal against the order of the Income Tax officer. there is delay in filling appeal and Two years is over against the order passed. Suggest me procedure for condonation of delay alongwith affidavit format. Pls. reply as early as possible.

An assessee may go for appeal against an IT order to CIT(appeal) within a time limit of 30 days from receiving the order. The CIT(appeal) shall condone the delay provided a valid ground has been given by the assessee. Submit appeal in form 35 with a court fee stamp.
