Is the application format for Certified copy is same as that in the given link https://icaiexam.icai.org/inspection/declaration.php
Or do we need to Write a separate declaration & Application & scan it?

Format given in the link is the correct format.
But, the application should be handwritten ( in the same manner as shown in the link)
