while filling examination form certificate of service is required to be attached,if I have already completed article training than there is any requirement to obtain new certificate of service from CA each time,

Only your own self attested Certificate of Service is required
if you have completed your articleship then you should download the certificate of service (completed articleship) and get it self attested
