1. Acknowledgement of Online submitted form along with the printout of the originally submitted form.

2. Self Attested Copy Of PAN Card.

3. Proof of the Constitution of Business:-

• Partnership Deed

• Certificate of Incorporation, MOA & AOA

• Any other Proof

4. Proof of the Residence of the Partners, Director & Proprietor (Provide 2 proof each person):-

• Passport

• Driving License

• Election Card

• Aadhar Card

• Ration Card

• Electricity Bill

5. Proof of the Place of Business:-

In case of Ownership-Provide Proof of Ownership

• Electricity Bill

• Property Card

• Agreement of Builder

In case of Tenant- Proof of tenancy (either i or ii & iii)

• Leave and License agreement

• NOC letter

• Rent Receipt

Also Submit Document showing ownership of Licensor and his own identity proof.

6. Blank Cancelled Cheque.
7 .Shop and Establishment Certificate

8. Details of all the Partners or Directors such as:-

• PAN No.

• PTEC No.

In case of PTRC Registration also get details of No. of employees as per the following salary slabs:-

• Below Rs.5,000

• Between Rs.5,000 – Rs.10,000

• More Than Rs.10,000

I need details and procedure for cancellation not for registration.
