I haven't give Advance itt test due to Covid-19 as tests were not conducted during the lockdown. So am I eligible to fill the exam form? If yes, which option should I choose where it is asked whether I have given advance ITT exam?
I have completed 2 1/2 years of my training. Should I attach decleration of service with my form? If yes, then please provide the format as I was not able to find any format which suits my condition in the sample formats provided by ICAI.

You should apply for Adv itt test virtually. Exam will be held from 15th Aug to 20th Aug.

Virtual Adv ITT exam will be conducted from 15th August. Unfortunately, to enroll for examinations the windows are now closed. Hope the gate re-opens, if most students were unable to enroll.

Advance ITT exam will be held in September again; applications for the same can be filled now. Last date is September 11, 2020.
