C form

I have supplied goods during the period 2011-12 to Surya Pharmaceuticals Ltd., Jammu under Form C. The company is closed since then. We have received a notice from Sales Tax office to submit Form 'C'. As the company is closed all their telephones are disconnected and we are not able to talk to anybody. Please let me know the procedure to close the file.
