You dont have to fill Form 103 again. Only u have to do is write an application to the institute mentioning the address of coommunication of the same city where ur CA office is situated i.e. Patna on letter head of your CA & signed by ur CA.

Also attach DD of Rs. 2000/- with it as ur fee for articleship is pending. 

Speed post it soon at Kanpur. Delay can prolong ur exams .

I live in patna on rent on lodge,lodge owner is not residing there,if any letter/notice will be send to that address, no body can receive,i think that I mention office address for communication address,but sir bol rahe hai ki nhi kr sakte,
