A few doubts - filling IPCC Registration Appln form

I have the following doubts regarding filling of IPCC Registration Application form

- What's the State Code of Karnataka.(Its not mentioned in the prospectus/Appln form)
- I have done my B.Com(it comprises of 6 semesters).What should be the marks obtained and max marks. Is it that of 6th sem only or aggregate of all sems
- Do i have to enclose any documents with the same? (Not mentioned in the prospectus/Appln form given but in appln form downloeded its mentioned to attach 12th marks card and attested Downloaded CPT Marks card)

u need to take print out copy of cpt marksheet n should get attestation........

code for karnataka  is kr ,

12 th marks card ,cpt  internet printout ,10 th marks card ,,,,,all attested by ca   ,,,,,,,i had called icai bangalore.........

I want to know the State Code of Gujarat which to be filled up in IPCC form. Also tell me is there is a requirement of any CA on IPCC filleed up form before submitting it.

no need for any ca attestation on ipcc form but on marks card attestation is a must
