Login to https://incometaxindiaefiling.gov.in and "request for refund re issue" by declaring your problem.
NO REFUND FAILURES TO RAISE REFUND RE ISSUE SERVICE REQUEST shown by income tax site
Wait for cancellan of cheque (3 months) or Submit a request letter to the issued Bank for cheque cancellation, post that try with refund re-issue request.
